(1) If the child to be vaccinated appears to be unwell, the persons mentioned in rule 10 must provide a certificate of unwellness by opening the following matters-
(a) Cause of ill health,
(b) Period,
(c) Name of the child,
(d) Year,
(e) Religion,
(f) Gender,
(g) The name of the municipality or village development committee where the child resides (h) Name of Patron,
(i) Full address of the custodian.
(2) The person issuing the certificate as per sub-rule (1) may set a date for the purpose of vaccination, stating that the child to be vaccinated can be healthy in so many days and if he is healthy, he can be vaccinated after so many days.
